How to Request Your Medical Records

Patients have the right to access their medical records under the Health Insurance Portability and Accountability Act.
Step 1: Complete a Medical Records Request Form
To request your records, please complete our Authorization for Release of Information form.

- Or stop by one of our offices and fill out the form in person
Step 2: Provide Required Information
Please make sure your form includes:
- Full name (and any previous names)
- Date of birth
- Phone number and/or email
- Specific records requested (example: dates of service, imaging, labs)
- Delivery method (email, fax, or pick-up)
- Signature and date

Information Needed
The Board requires a completed, signed complaint form containing:

- The name of the chiropractor or facility involved.
- Specific details of the complaint (date, time, and description of the incident).

File a Consumer Complaint
To file a consumer complaint in Texas, submit an online complaint via the Texas Attorney General's Consumer Complaint Portal. For specific industries, file with the appropriate state agency, such as the Texas Department of Licensing and Regulation or the Texas Department of Banking.
